Infertility

Approximately 6,000,000 U.S. women are presently diagnosed with infertility. Ovulatory dysfunction and mechanical factors are the major causes of female infertility. Approximately 30-50% is due to mechanical factors such as blocked fallopian tubes, scarring, or adhesions and 40% are due to ovulatory factors.

The primary mechanical cause of female infertility is the adhesions & scar tissue that form in or around the uterus, ovaries, fallopian tubes or fimbriae, and accompanying vertebral/pelvic joint restrictions.

Mechanical causes of infertility can result from complex conditions such as endometriosis or pelvic inflammatory disease. Abdominal or pelvic surgeries, falls, trauma, and infections, tubal damage, ruptured ovarian cysts, bowel or bladder dysfunction also cause adhesion formation. Female infertility can also be accompanied by pelvic, rectal, and low back pain along with other pelvic floor disorders such as incontinence or constipation.

When adhesions form on female reproductive organs they can block fallopian tubes or adhere to the uterus or ovaries. Adhesions can attach to the finger-like fimbriae of the fallopian tubes, preventing them from grasping the egg during ovulation. The fallopian tubes must have complete freedom of movement to transport the egg to the uterus. Adhesions that form between muscle cells within the cervix or uterus can make sperm transport difficult and implantation of a fertilized egg difficult.

Manual orthopedic and visceral physical therapy specifically addresses each of the mechanical dysfunctions involved. This includes the appropriate vertebral/pelvic joint restrictions, ligamentous restrictions, and adhesions that cause poor reproductive function.   • What are adhesions and scar tissue?
    • The abdominal cavity and reproductive organs are in constant movement and should be able to glide over themselves, the side wall of the body and to glide over the other organs without any friction. In order to have healthy organ motility without friction the side wall of the body and organs are covered with a special layer or membrane called the peritoneum. Fluid is present between these layers to reduce friction. Injuries to the peritoneum (surgical cuts, infections (infections of the fallopian tubes included) and endometrial lesions) effect the layers of peritoneum. The layers become rough and sticky and where the two layers come intact, they stick together.
    • Adhesions begin when tissue that should be separated grow together and adhere to each other; often as a result of infections or surgery or trauma. Adhesions over the ovary may interfere with ovulation. Adhesions and micro adhesions are desposits of scar tissue that form as a normal response to tissue injury caused by infection, inflammation, surgery or trauma.
